English Meadows offers scheduled transportation services every Monday and Thursday for residents who need assistance traveling to appointments or errands. Transportation is required to be booked in advance, with pickup and drop-off times available between 8:30 AM and 4:00 PM.

Transportation within a 10-mile radius is included. For destinations beyond 10 miles, a mileage fee of $0.50 per mile will apply.

Lavender Hills residents will require a family member to meet them at the appointment. If a family member is not available, arrangements must be made for an English Meadows associate to accompany the resident. This support is provided as a 1-on-1 service and will incur an additional fee.

Residents are encouraged to schedule early to ensure availability. Our goal is to provide safe, reliable, and comfortable transportation to support the needs of our community.

Care Conference

A care conference is an opportunity for families, residents, and our care team to come together for a collaborative and meaningful conversation about your loved one’s well-being. During this meeting, we will review current care needs, discuss any changes or updates, and ensure that the care plan continues to reflect your loved one’s preferences, strengths, and goals.

Our team will provide updates on health, daily routines, engagement, and any recent observations. Families are encouraged to share questions, concerns, or insights so we can work together to support the highest quality of life for your loved one.

The goal of a care conference is to promote open communication, strengthen our partnership with families, and make sure your loved one receives compassionate, individualized care every day.

Whether you’re hosting a birthday lunch, enjoying a private visit, or gathering for a special occasion, our Private Dining Room can be reserved in advance to ensure dedicated time and attention.
